I, the undersigned, hereby represent that I am of legal age and hereby release and forever discharge Repertory Dance Theatre (RDT), Salt Lake County and all members of said organizations and all agents, representatives, employees, and independent contractors of said organizations and their successors from all claims and demands which I, my heirs, executors, and administrators, have or may have against said organizations by reason of participation in classes conducted by RDT or RDT's Dance Center on Broadway, and assume all risks in such activity.

Additionally, you give permission for images taken during class through videos, photos, or digital camera to be used solely for the purposes of RDT's promotional material and publication and waive any rights of compensation or ownership there to.

We have taken enhanced health and safety measures for you, our other Guests, and teachers. You must follow all posted instructions while visiting Rose Wagner Performing Arts Center. An inherent risk of exposure to COVID-19 exists in any public place where people are present. COVID-19 is an extremely contagious disease that can lead to severe illness and death. According to the Centers for Disease Control and Prevention, senior citizens and guests with underlying medical conditions are especially vulnerable. By visiting Rose Wagner Performing Arts Center, you voluntarily assume all risks related to exposure to COVID-19. Lets keep each other healthy and safe.

If you exhibit any symptoms of COVID-19 or have been exposed to anyone with the virus, please stay home. 

COVID symptoms may include fever or chills; cough; shortness of breath or difficulty breathing; muscle or body aches; new loss of taste or smell; sore throat; congestion or runny nose; and nausea, vomiting, or diarrhea.

I am over 18 or the legal guardian of the participant above and I accept the terms of the waiver.
